Job SummaryThe University of New Mexico - Hospitals seeks a skilled and compassionate RN Care Manager to join our team. As a key member of our Care Management Services department, you will play a vital role in coordinating patient care and ensuring the highest quality outcomes.Responsibilities

• Coordinate patient care and develop comprehensive plans to meet individual needs
• Collaborate with healthcare teams to ensure seamless transitions and efficient resource utilization
• Assess patient clinical, psychosocial status, and current treatment plans
• Provide education and support to patients and families to promote health and wellness
• Ensure adherence to hospital and departmental policies and proceduresRequirements
  • 1 year of direct experience as a registered nurse
  • RN licensure in New Mexico or reciprocal agreement
  • CPR for Healthcare/BLS certification
  • Bilingual English/Keres, Tewa, Tiwa, Towa, Zuni, or Navajo (non-essential)Working ConditionsThis is a light work position, requiring occasional lifting of up to 20 pounds and frequent sitting. Minor hazards include physical risks, dirt, dust, fumes, and noise.
